I found a very effective way to cool down the hottest spot on pi 5 - the PMIC.
Works beautifully with the ICE Tower cooler.
Used 2x3mm stacked Thermal Pads (high quality Gelid Extreme) and a small aluminum heatsink:

The result - 50C max under heavy stress test and OCed CPU/GPU 3GHz/1.15GHz.
